A 78-year-old man with end-stage renal disease and microscopic polyangiitis, for which he was taking glucocorticoids, presented with fever, taste abnormalities, and a painful ulceration of the tongue. Biopsy of the lesion confirmed a diagnosis of cytomegalovirus infection.
